Ex-Servicemen Contributory Health Scheme (ECHS) – Latest List of ECHS Empanelled Hospitals

Ex-servicemen Contributory Health Scheme (ECHS) was launched with effect from 01 April 2003. The Scheme aims to provide allopathic and AYUSH medicare to Ex-servicemen pensioner and their dependents through a network of ECHS Polyclinics, Service medical facilities and civil empanelled/Govt hospitals/specified Govt. AYUSH hospitals spread across the country. The Scheme has been structured on the lines of CGHS to ensure cashless transactions, as far as possible, for the patients and is financed by the Govt of India.

ECHS is a flagship Scheme of the Ministry of Defence, Department of Ex-Servicemen Welfare. The aim of Scheme is to provide quality healthcare of Ex-servicemen pensioners and their dependents. As on 01 April 2019, an estimated 55 laksh beneficiaries including dependents are being served by this Scheme.

There are 427 Polyclinics in 28 Regional Centres sanctioned by the Government across India providing health care facilities to the Ex-Servicemen and their family members. ECHS Polyclinics are designed to provide ‘Out Patient Care’ which includes consultation, essential investigation and provision of medicines.

The Central Organisation ECHS is located at Delhi and functions under the Chiefs of Staff Committee through the Adjutant General in the Integrated Headquarters of Ministry of Defence (Army). The Central Organisation is headed by a Managing Director, a serving Major General.

FAQ on Ex- Servicemen Contributory Health Scheme (ECHS)

There are 30 Regional Centres sanctioned by the Govt, out of which 28 Regional Centres are functional. All efforts are being made to make the 2 newly sanctioned Regional Centres at Yol and Bhubaneshwar functional at the earliest possible.
ECHS Polyclinics are designed to provide ‘Out Patient Care’ which includes consultation, essential investigation and provision of medicines. Specialized consultations, investigations and ‘In Patient Care’ (Hospitalization) is provided through spare capacity available in Service hospitals / Government Hospitals / civil hospitals empanelled with ECHS.
Polyclinics are categorized as Type A to E, based on the number of Ex-servicemen residing in that area.
They are:-
Type A – Above 20,000
Type B – Above 10,000
Type C – Above 5,000
Type D – Above 2,500
Type E (Mobile) – Less than 2500 (Armed Forces Polyclinics or augmented Armed Forces Clinic will be provided)

FAQ on ECHS Online Smart Card

Eligibility For ECHS Membership:

The Scheme caters for medical care to all ESM pensioners including those earning Disability and Family Pension and their dependents, which includes wife/husband, dependent legitimate children and wholly dependent parents. To be eligible for membership of ECHS, a person must meet the following twin conditions:-
Should have ESM status.
Should be drawing normal Service /Disability/ Family Pension.

Revised Eligibility Criteria:

Govt sanction has been accorded for bringing ECHS beneficiaries at par with that of CGHS. Details of Relationship and Criteria are as under:-

Son – Till he starts earning or attains the age of 25 years, whichever is earlier.
Daughter – Till she starts earning or gets married, irrespective of the age limit, whichever is earlier.
Son or daughter suffering from any permanent disability of any kind (physical or mental) – Irrespective of age limit.
Dependent divorced/abandoned or separated from their husband/ widowed daughters and dependents unmarried/divorced/abandoned or separated from their husband/ widowed sisters – Irrespective of age limit.
Minor brothers(s) who are dependent on the veteran – Upto the age of becoming a major
Parents – Fully dependent

PROCEDURE OF ONLINE APPLICATION FOR ECHS CARD

Preparation

1. Scan the following and save each document in separate file of JPEG,of 1 MB max, for uploading :-

1. Pension Payment Order (PPO) of Primary Beneficiary, i.e., Ex Service MAN (ESM) if he is alive or his Wife, if ESM is not alive.
2. Old ECHS Card (front side only) / Temporary Slip of Primary Beneficiary
3. Photo of Primary Beneficiary (Passport size colour photo in white background)
4. Signature in Blue Ink, of Primary Beneficiary
5. Photos of Dependants (Passport size colour photo in white background)
6. Signatures in Blue Ink or Thumb Impressions of Dependants
7. Death Certificate of ESM, if he is not alive
8. Disability certificate of ESM, if disability is battle disability.
9. Disability certificate of dependent(s) if dependent(s) have disability
10. For first time application of pre 2003 retiree:-
(a) DPDO/ Bank certificate, certifying that FMA (Fixed Medical Allowance) has been stopped.
(b) MRO receipt for ECHS membership fee, for those retired from 1996 to 2002.

2. Keep the following ready, for information to be filled in the Application:-

1. Aadhaar and PAN Cards of Primary Beneficiary &Aadhaar Cards of all dependants
2. PAN cards of dependents if available
3. Bank Pass Book of pension account & ESM Discharge Book
4. Permanent and Temporary address of Pensioner
5. Blood group of ESM and all dependents
6. Debit or Credit Card and it’s linked phone to get OTP and make payment of Application Fee online (Rs 177/- per card)
7. Email Id, if available, for retrieving Password etc.

ECHS Smart Card Online Registration

1. Go to web site https://echs.gov.in and Click “OK”
2. Select “ ONLINE SMART CARD APPLICATION”
3. Select “ ONLINE APPLICATION” and Click “OK”
4. Then, Register Application, keeping the following points in mind:-
1. If there is no Prefix, leave it blank &. If there is no suffix, Put’ $’, in the space.
2. Enter Mobile Number, which should be active to receive OTP immediately and all the messages regarding the ECHS Card subsequently.
3. Create your password. Keep this Mobile Number and Pass Word with you, till your ECHS Card is received, so that you can see the progress and observations on the application, if any, during the processing of ECHS Card.
4. Enter Mother’s Name & Place of Birth of Pensioner. Note down these with correct spelling, including Capital/ Small letters. These can be used to retrieve/ change password.
5. Click “REGISTER”. Message will appear as “Registration done successfully”. Click OK.
6. Login page will open. Login using Mobile No and Password given. OTP will be sent to Registered Mobile and mobile verification page will open. Enter OTP and click “Verify”.
7. Message of “MOBILE NUMBER VERIFIED” will be displayed.

Filling the Application Form Online

1. Go to web site https://echs.gon.in. Click “OK”
2 Select “ ONLINE SMART CARD APPLICATION”
3. Select “ ONLINE APPLICATION”
4. Click “OK”
5. Click “ALREADY REGISTERED. CLICK TO LOGIN”
6. Select the Application Category by clicking “Drag down Icon, small arrow”
7. Fill in the EX SERVICEMAN (ESM) DETAILS.
8. Upload Photo & Signature of Primary Beneficiary
9. Enter other details of Primary Beneficiary
10. Upload Photos/ Signatures and other details each dependant.
11. Upload PPO, Old ECHS Card of Primary Beneficiary
12. Upload Death Certificate, if ESM is not alive
13. Upload Temporary Slip and other documents, if applicable.
14. Agree the SELF DECLARATION by clicking “ I AGREE”
15. Save the details entered by clicking “SAVE DETAILS”
Saved details can be modified or corrected if necessary by logging in once again and saving the corrected details.
16. Make Payment by Clicking “MAKE PAYMENT” and giving other details asked.
17. Take Print out of Application Form, Payment Receipt and Temporary Slip, by logging in, after Payment of Fee. Note down Phone Number, Pass word, Mother’s Name and Place of Birth at the end of Application printed, for future reference.
18. Get the Signature of OIC ECHS on the Temp Slip, Which can be used as ECHS Card, till you receive the NEW ECHS CARD.
19. Check for Observations if any from Record Office, by logging in frequently till the application is verified by Record Office concerned. Sort out the observations by suitable actions and change your details by editing your application, if necessary. Save details once again after editing the application.
20. Wait for SMS giving OTP for collection of NEW ECHS Cards, to be collected from Station Headquarters.
